The Healthy Heart Program is a telephone-based support and coaching program for members living with heart failure.

Specialist nurses provide advice to help you take an active role in managing your own care, staying well and out of hospital.

If you have been diagnosed with Heart Failure, or suffer from chronic symptoms such as unexplained weight gain, shortness of breath on exertion or at rest, or swelling in your ankles, (which may be indicators of heart failure) you may be eligible for the program.
