1. $100 Gift Card to be provided to every current Australian Unity member whose recommendation results in a new member. This offer excludes new members who join extras only cover and members on Overseas Visitors Cover.
2. An existing member is only entitled to receipt of one card per recommended new membership of Australian Unity Health Limited. Subsequent to joining, a new member can introduce new members and receive a $100 Gift Card.
3. The recommending member details must be included on the new member application form for the Gift Card to be issued.
4. The Gift Card offer is only available to current, financial members. Members whose membership is suspended, in arrears or unfinancial are not eligible.
5. This offer is current as at June 2009, and can be altered or withdrawn at any time. Australian Unity reserves the right to change the offer conditions at any time.
6. This offer excludes all other promotional offers. This offer is not available to members on specially negotiated corporate plans. This offer is not available to Australian Unity sales staff.
7. All cards will be delivered by registered post. Australian Unity cannot be held responsible for any damaged, undelivered, lost or stolen Gift Cards and no replacement will be provided. Allow six weeks for delivery of the Gift Card. Gift Cards are subject to the terms and conditions that appear on the reverse of the Gift Cards.
