There is a strong link between having a healthy weight and good health. Australian Unity will help you on your way by providing great incentives for you to achieve your weight goals as part of your health insurance. You will be rewarded with up to $100 when you join an approved weight loss program, and when you reach your goal weight you'll receive another $100. If you stay within 5kg of your goal weight for 12 months, Australian Unity will reward you again with a further $150.

Approved programs include:

• Weight watchers (excluding at home kits)
• Sure Slim
• Jenny Craig
• Ultralite

To claim, you will need to supply the receipt from an approved weight loss supplier with a completed claim form to Australian Unity. To receive benefits for reaching your weight goal and weight maintenance, a certificate of goal achievement endorsed by your weight loss provider must be supplied for both occasions with a completed claim forms.

• Up to $100 per person on joining an approved program applies to membership fee only, not food or products
• $100 per person on your weight loss goal achievement
• $150 per person after 12 months if you are with in 5kg of your goal weight
• $550 per person lifetime limit
• A valid receipt on joining and certificates of achievement are required when claiming

Available with Hospital, Combination and Extras covers except Care 'n Repair and Overseas Visitors Cover. Where a member has both Hospital and Extras cover benefits may only be claimed under the Hospital cover.
