My Health Guardian
Support if you have a chronic condition.
Care support services available include:
- Telephone support from registered nurses
- A registered nurse care manager to help you create an online health action plan to monitor your progress
- Online access to approved health coaches for guidance and advice on your diet, fitness and lifestyle
- SMS or email reminders for any health tests you might need
- Reminder calls prior to your GP appointments
- Advice on local community services to help you maintain your condition
To join the My Health Guardian program, you need:
- To have a chronic condition
- HCF hospital health insurance cover (Extras only, Ambulance only or HCF Life only products are not eligible)
- To be at least 18 years old
While you are in the program, HCF will keep your GP informed of your health status (NOTE - the program is not intended to replace your GP’s treatment plan).
