HCF Health Insurance
The HCF Healthy Weight for Life Type 2 Diabetes Pilot Program
To be eligible to participate you:
- have been medically diagnosed with Type 2 Diabetes, have a body mass index (BMI) > 27 and you are under medical supervision
- have an HCF hospital product as part of your health insurance for at least 12 months
- have obtained approval from your medical practitioner to participate
- have no allergies or intolerances to KicStart ingredients (see KicStart™VLCD information)
- are not pregnant or breastfeeding or intend to become pregnant during the 18 week program
- are willing to provide data over the course of the program on: height, weight, waist circumference, HbA1c, blood pressure, cholesterol, related mediaction changes
- have regular internet access and a personal email address for receiving follow up information and reminders
- are 25 years and over
- are motivated and able to complete the 18 week program
