• HCF will only contact you if you register your interest to participate on the website or by calling HCF on 13 13 34.
• PHS will only have access to the information you provide to them.
• Data collected from you will only be used for the specific purpose of delivering and evaluating the HWFL Heart and Diabetes program.
• PHS has complied with HCF’s Privacy Policy and is bound by a signed confidentiality agreement to ensure all members’ personal information is secure and kept in strict confidence.
