Welcome to the Total Care Program

The Total Care Program provides support for people with chronic or complex illnesses or conditions. If you are eligible for the Total Care program we provide you with a host of services, which include:

• Regular contact from an experienced Registered Nurse.
• A phone-in service to our Registered Nurses for those
  unexpected questions.
• A comprehensive book on how you can live a healthy life with
  a Chronic Condition.
• Links to websites for information and support about your condition

Regular Contact

We'll contact you at regular intervals to assist you to self-manage your condition. We can provide information on your condition, and access to support groups and services.

Am I eligible?

You can enrol on our Total Care Program if you are an ahm member and:

• Have a Chronic or Complex illness.
• Have Hospital Cover (with or without a co-payment) with the fund. This means Hospital Cover, Hospital Cover Level 5 or Hospital Cover Level 8. 
• Have served your waiting periods.
